Claims

For accident claim and medical/hospital cash claim submissions, you can click here to submit new claim by e-form.

If you wish to make other life policy claim(s), please call our Customer Care Hotline on +852 2535 3502 or click here to enquire. To facilitate processing, please submit all required documents with the claim form.

  • 1. Critical illness/Female Product claim

    Additional information: Completed appropriate "Attending Physician Statement" and all medical evidence copy issued by hospital and doctor

  • 2. Death/Payor benefit claim

    Additional documents required:

    1. Original Death Certificate

    2. ID Card copy of the deceased

    3. Certified true copy of ID Card of the beneficiary

    4. Authorization for obtaining medical report

    5. Original Policy document or Lost of Policy Declaration

    6. Letter of Administration if the policy has no designated beneficiary and there is no will left, or probate if there is a will left

    7. Certified true copy of address proof of beneficiary/administrator issued within the last three months (e.g. gas, water, electricity, phone bill for land line or bank statement)

    8. Relationship proof copy between the life insured and the beneficiary
